ADDITIONAL INFORMATION... We provide a variety of services right in the office such as X-Ray, theraputic ultrasound, minor surgery and mycology lab testing. All major insurance carriers are accepted and we are affiliated with all PPOs and most HMOs with a referral.  All major credit cards and cash accepted.  Visits start at $45.00.
